For most educator license applications listed below, you may now renew or apply for your license in just a few minutes online if the person who approves licenses at your district or university is signed up as an electronic signer.
Authorized representatives – (superintendents, local professional development chairs and university deans or their designees) will find instructions on how to complete the e-signer registration process here.
Licensure applicants – Access or create your SAFE account to get started on the easy online application process through My Educator Profile. Once you have a SAFE account you will be able to access My Educator Profile from the ODE CORE link on your SAFE account menu. If you like, you may click here for informational videos and the My Educator Profile User Guide for more information. If your application is not available for completing online yet, it soon will be. Please note that initial out-of-state licensure requests DO NOT require an authorized e-signer.
Beginning January 2014 – all first-time and renewal licensure applications must be completed online.
Until January, applications for licensure also will remain available for download via the links below. Applications are in PDF format, and require Acrobat Reader to view and print. If you do not have the Acrobat Reader installed on your system, you may download it free here.
1. 4-Year Resident Educator License (education program completed in Ohio)

2. Initial License (education program completed inside of Ohio)
This application may be used for the following purposes:
- An Ohio college or university recommending eligible individuals for an initial Ohio license, and to add new areas to existing licenses.
- Individuals applying for an initial Physical Therapist, Occupational Therapist, Physical Therapist Assistant, Occupational Therapist Assistant license.
3. Initial License (education program completed outside of Ohio)
This application may be used by individuals who have completed licensure programs at colleges/universities outside the state of Ohio. Please read all of the instructions and information prior to completing and submitting the application. Please be sure to download the Instruction Packet for Out-of-State applicants. If you live outside the state of Ohio, you may obtain the BCII and FBI criminal background report fingerprint cards by calling the Office of Educator Licensure at (614) 466-3593.
4. 2-Year License Renewal or Advance
This application may be used for the following purposes:
- To renew a 2-year license
- To advance a 2-year provisional license to a 5-year professional license
- To advance a 2-year alternative license to a 5-year professional license

Please note: If you will be a participant in the Ohio Resident Educator Program, you will not be able to register for this program if you hold a two year provisional license. Please use application #2 above to apply for a four year resident educator license, rather than renewing your provisional license.

5. 5-Year License Renewal or Transition
This application may be used for the following purposes:
- To renew a five-year professional or associate license
- To transition a 4-year or 8-year certificate to a 5-year license
6. Advance to Senior Professional Educator License or Lead Professional Educator License
7. Pupil Activity/Coaching Permit
The pupil activity permit is required for a pupil activity program that involves athletics, routine or regular physical activity, or activities with health and safety considerations.

8. Substitute License - Long-Term and Short-Term
Please note that applications for these licenses need to be initiated by an employing Ohio school district.
9. Educational Aide Permits and Monitor Permits
Please note that applications for these permits need to be initiated by an employing Ohio school district.

10. Supplemental Teaching License Application
This application may be used for the following purposes
- To request a supplemental license
- To renew a supplemental license
- To progress from the supplemental license through a State Equivalent Review
11. Evaluation Request for Licensure through the Supplemental Pathway
This evaluation may be requested only by teachers who hold a supplemental license and are requesting a state equivalent review for full licensure. Teachers who are enrolled in an approved program of preparation at a college or university would continue to follow the college's program. An evaluation would not be needed.
12. Adult Education Teaching Permit
These applications need to be initiated by the employing school district.

13. Evaluation Request Application for the Alternative Resident Educator Pathway
This application may be used to request an evaluation for Designated Subjects grades K-12, Intervention Specialist K-12 or World Languages P-12. Applicants who hold a professional license or certificate are not eligible for an alternative evaluation.
When completing the application please reference the appropriate file below to retrieve the teaching code(s) requested.
Career-Technical Workforce Development applicants are not eligible for an alternative evaluation and should consult with their employing school district concerning their qualifications for an alternative license.
14. Alternative Resident Educator License
This application may be used to apply for an alternative teaching license for:
- Designated Subjects grades K-12
- Intervention Specialist grades K-12
- World Languages grades P-12
- Career-Technical Workforce Development
15. Alternative Principal License
This application may be used for the following purposes
- To request an alternative principal license
- To renew the alternative principal license
- To advance from the alternative to the standard principal license.

16. Alternative Superintendent and Administrative Specialist License
This application may be used for the following purposes
- To request an alternative superintendent or administrative specialist license
- To renew the alternative superintendent or administrative specialist license
- To advance from the alternative superintendent or administrative specialist license to the 5-year professional license.

17. 5-Year School Treasurer and School Business Manager Licenses
This application may be used for the following purposes.
- To request a school treasurer or school business manager license
- To renew a school treasurer or school business manager license
18. Temporary License
This application may be used only by employing schools/districts when requesting full-time temporary licenses for pupil services, military science, 12-hour teaching permits and 40-hour STEM School teaching permits. This application must be initiated by an employing Ohio school district.
19. Non-Tax Certificate
This application may be used only by an employing Ohio private non-tax supported school to recommend individuals for a non-tax certificate.

20. Duplicate or Name Change Request
This application may be used to request a copy or name change of your current license.
